Definition of ROOT CAUSE ANALYSIS

Organization faces many issues with regard to its operational processes, quality, productivity, reputation, etc. Under these circumstances, root cause analysis is undertaken before defining the corrective action that could taken (be it short-term, long-term, or both). Root Cause Analysis (RCA) is one of the most important problem-solving and quality improvement approaches undertaken by organizations to identify, understand and resolve root causes of problems and incidents. RCA process involves adoption and application of a series of ‘Best-Known-Methods (BKM)’, which when used in different ways, can produce systematic, rational, quantifiable and documented approaches to identify, understanding and resolving underlying causes of under-achieved quality in organizations.
Root cause analysis processes have been adopted successfully in thousands of organizations; by top executives, managers, and team leaders; across all industries, government agencies, all over the world. - Define problem effectively, and clearly distinguish between the root-causes and effects.
- Collect and analyze data; use K-T Situation Analysis technique to appraise situation.
- Apply the following RCA techniques & tools to resolve problem & preventing recurrences:
– Pareto Analysis.
– Why-Why Analysis
– Cause & Effect charting
– IS/IS NOT Logic (Problem Analysis)
– Fault Tree Analysis
- Formulate corrective action plan to resolve the issue.
- Experience the Human-Side of problem solving, teamwork and the vital role communication plays at each step of the process.
