COURSE DESCRIPTION

Definition of  ROOT CAUSE ANALYSIS
“The systematic process of finding the knowledge
or best practices needed to prevent a problem.”

Organization faces many issues with regard to its operational processes, quality, productivity, reputation, etc. Under these circumstances, root cause analysis is undertaken before defining the corrective action that could taken.  Root Cause Analysis (RCA) is one of the most important problem-solving and quality improvement approaches undertaken by organizations to identify, understand and resolve root causes of problems and incidents, before the situation become worse.  RCA process involves adoption and application of a series of ‘Best-Known-Methods (BKM)’, which produces systematic, rational, quantifiable and documented approaches to identify and resolving underlying causes of under-achieved quality in organizations.

Root cause analysis processes have been adopted successfully in thousands of organizations across all industries, government agencies, all over the world.   When you follow these best practices, the results are powerful; RCA training helps eliminate challenging problems, prevent issue recurrence, achieve significant return on investment, create a culture of logical problem solving, foster effective goal setting, help improve communication and facilitate better planning, clearer thinking and faster fault resolution. 

This remote online training (ROT) program examines step-by-step ‘how-to’ processes for achieving that. Included are best practices tools appropriate for moving through each step of the process.  It is interactive and engaging with break-out group discussions, interactive board brainstorming, case studies, quizzes and analytical exercises.

 

LEARNING OUTCOMES

At the end of the course, participants will be able to:

    • List the step-by-step analytical and rational Best-Known-Methods (BKM) process to solve problems.
    • Define problem effectively, and clearly distinguish between the root-causes and effects.
    • Collect and analyze data; use K-T Situation Analysis technique to appraise situation.
    • Apply the following RCA techniques & tools to resolve problem & preventing recurrences:
        – Pareto Analysis.
      – Why-Why Analysis
      – Brainstorming
      – Cause & Effect charting
      – IS/IS NOT Logic (Problem Analysis)
    • Formulate corrective action plan to resolve the issue.
    • Experience the Human-Side of problem solving, teamwork and the vital role communication plays at each step of the process.

 

 

CERTIFICATION

No Certification.

Certificate of Achievement (for those who scored 80% and above for post-test) or Certificate of Attendance.

2-Days (9.00am – 5.00pm Local Time) or Total = 14.0 hours
